I regret that I cannot make up time if you are late. It is important that you read, understand these terms and conditions before you proceed with counselling, coaching or therapy.

CANCELLATION

If you need to cancel your appointment, please give 48 hours notice or you may be charged the usual fee. Weekends are, of course, not included in the 48 hours cancellation window. Example, Mondays or Tuesdays appointments need to be canceled at latest on Wednesdays/Thursdays as 48 hours prior appointment but excluding the weekends days. . Failure to give the required notice will result in you being charged a cancellation fee.

AGE LIMIT

Children younger than 18 years old need consent from their parent or guardian.

CONFIDENTIALITY

Overall, what is said between you and me in the therapy sessions is strictly confidential.

Payment

All fees, costs and other remuneration of the services provided by The Hague Psychologist/Leiden Psychologist/Expat Psychologist, and agreed orally and/or in writing must be paid either in cash after each session or within two weeks.

Complaint

Expat psychologist/The Hague Psychologist strives make the complaints process as accessible and as transparent as possible. To get an initial clear picture, we recommend sending an email. Dissatisfied? Talk to each other first!

Complaint outside practice

Private International Psychological Practice / Expat Psychologist strives to handle and resolve complaints as soon as they arise. If this does not lead to a satisfactory solution for both parties, a complaints officer can be contacted.

Not satisfied with your counselor? 

Something can go wrong during treatment or counseling. If, in your opinion, you have been treated or supervised negligently or incorrectly, you can submit a complaint. 

It is important that you first try to discuss and resolve the complaint together with your healthcare provider. 

It is also possible to engage the complaint officer * of the NFG. 

If you are unable to resolve the complaint, you can submit a complaint further. Because of the Wkkgz, the NFG's own complaints regulations have lapsed as of 1-1-2017. Every professional care provider is responsible for ensuring that the Wkkgz legislation is in order. According to the Wkkgz, your care provider is obliged to tell you how he / she has arranged complaint handling. Your care provider is affiliated with Quasir via the NFG. Your complaint must be handled according to this schedule.
  1. You are not satisfied with your care provider.
  2. Talk first about your complaint with your care provider.
  3. If you can resolve complaint together, you can let it go.
  4. If you are still not satisfied you can report your complaint with the independent Complaint officer*. Also, for the details of the complaints officer you can contact the NFG secretariat via the email address info@de-nfg.nl.
  5. Your complaint will be resolved professionally, you can let it go.
  6. If you are not satisfied still you can report your complaint in a written form with your care provider. You will received during 6 weeks (can be prolong also with 4 more weeks) a judgement.
  7. If you are still not satisfied you can report your complaint with Geschilleninstantie Zorggeschil - Quasir - https://www.quasir.nl/.
  8. Quasir will make a binding decision.
  9. You are satisfied with the decision, you can let it go.
  10. If you are still  not satisfied with the resolution of your complaint you can approach sub-district court judge.

If you are unable to resolve the complaint, you can submit a complaint to the complaint officer.

 

A complaint is a signal from a client about the care and services of Private International Psychological Practice. Every complaint will be taken seriously and addressed to improve the satisfaction of the treatment of the client.

 

Private International Psychological Practice strives for an easily accessible complaints procedure and aims to use complaints to improve treatment and care immediately. This means that: It is clear to the client, where and how a complaint can be expressed; Dissatisfaction and complaints should be discussed, where possible, directly with the directly involved employee so that any problems at that level can be resolved; Information and an open attitude towards complaints and suggestions are important to all concerned.

 

Private International Practice is based on mutual respect between client, family member or close relative and employee and tries to pursue equality as much as possible. The complaint must be filed, subject to special circumstances, within six months.

 

Quality: complaints are a manifestation of displeasure/discontent from an individual, but clients often give the motivating reason for complaint is with the intention that it lead to adjustments that make things better for new clients. Complaints, therefore, highlight the need for direct action from the petitioner.

We all possess the same needs for healthy agreements, boundaries, emotional expression and genuine connection. My approach accentuates these needs as normal, and teaches people how to meet them in healthy ways. We all want more peace and clarity in our lives. Major life transitions, good and bad, can rob us of both leaving us feeling overwhelmed and uncertain. Therapy can be an effective tool to help you heal and move forward. I believe building a trusting, respectful and collaborative therapeutic relationship can make it possible to do just that.

The safe harbor of the therapeutic relationship offers a chance for validation, honesty, acceptance, and change that is not always possible in other relationships Whether you seek support navigating a difficult life transition, managing or confronting difficult emotions, sorting out feelings about a relationship, understanding the effects of a painful past, or exploring ambivalence and fear of change, the therapeutic relationship can help you get “un-stuck” and moving forward. I’ve found most of my clients to be remarkably intuitive, insightful, and wise. Sometimes, due to the challenges of life and demands of relationships, people lose touch with their sense of self, their ability to trust their own instincts and their belief in their right to have their needs met.

What is the processing based on:

Your personal data will be processed in order to execute the agreement that we conclude for your psychological treatment. Your personal data can also be processed in order to comply with a legal obligation, such as, for example, obligations based on tax law. In addition, Taya Slobodjaniuk handles personal data for the legitimate interests listed below: improving services; protecting financial interests; securing and managing systems.

Provision to third parties

In the context of my services, personal data may be provided to third parties, such as the control of the professional association. These third parties may only process your data for the aforementioned purposes. Your personal data could also be provided in the context of a legal obligation or court order. I will never provide your details for commercial purposes.

Transfer of data outside the European Union

Your personal data can be processed outside of the European Union through Google Drive and Analytics services. Google is EU-US Privacy Shield certified and adheres to European privacy regulations.

How long do I keep your data?

I do not store your personal data for longer than necessary for the purposes stated in this privacy statement. This means that they are kept for as long as they are necessary to achieve the relevant goals. Certain data must be retained for a longer period of time, because I have to comply with statutory custody obligations, such as the fiscal retention obligation.

Privacy Statement about collecting data for psychologist practice

I am committed to protecting and respecting your privacy which means that I collect, use and disclose your personal information responsibly and only to the extent necessary for the services I provide.  This document describes my policies and procedures as they relate to collecting, using and disclosing your personal information. It also offers information about how you can access your records and request correction of recorded personal information.

Who I Am

Taya Slobodjaniuk, is the sole owner of the private practice, which delivers psychological services – Expat Psychologist, The Hague Psychologist, Leiden Psychologist, Student Psychologist, Private International Psychological Practice.

What is Personal Information?

Personal information is information about an identifiable individual.  It includes information that relates to an individual’s personal characteristics (e.g., gender, age, income, home address or phone number, ethnic background, family status); health (e.g., health history, health conditions, health services delivered to them), activities and views (e.g., religion, politics, opinions expressed by an individual, an opinion or evaluation of an individual). Personal information and specifically personal health information is protected by privacy legislation (i.e., the Personal Health Information Protection Act [PHIPA]), and is different from business information (e.g., an individual’s business address and telephone number), which is not protected by privacy legislation.

Collection of Your Personal Health Information

The purposes of collecting your personal and health information are to provide you with appropriate and quality psychological services, contact you for service-related reasons, and prevent harm (such as reaching an emergency contact). I will not collect information from you for any other purpose (such as conducting research) without first obtaining your informed consent. If you do not want to provide consent for the collection of this latter kind of information, you are completely free to refuse and there will be no impact on your services.

I will collect personal health information directly from you, except when you have provided consent for me to collect such information from others (such as a spouse, family physician, or mental health professional with whom you have previously worked), or when the law requires me to collect information without your consent (such as emergency situations where the purpose of collecting information is to prevent potential harm).

By law and in accordance with professional standards, I am required to keep a record of my contacts with and services to you. Your record includes information that you have provided to me or have authorized me to receive, such as consent forms, session notes, results of any assessments, billing information, contact records, and correspondence that I have sent to or received relating to your service. The physical records are the property of my practice; however, you have rights regarding access to and disclosure from your record (discussed below), regardless of the form in which the information is recorded. In this office, information is recorded in both written and electronic form.

If you contact me through my website I only retain the personal information you provide and only use that information for the purpose for which you gave it to me (e.g., to respond to your email message).

Use of Your Personal Health Information

Your personal health information is primarily used to provide you with psychological services such as psychological assessment or intervention. The delivery of psychological services includes such tasks as service planning, maintenance of records, monitoring, billing, and collecting unpaid accounts.  Other uses of your personal health information include to guide and improve the quality of services provided in my practice. Audits involve accessing and inspecting client records, however I do not permit any identifiable client information to be removed from my premises for the purpose of an external audit without your consent. In addition, all individuals involved in such an activity are professionals required by law to maintain the confidentiality of all information that is accessed. Finally, when psychological services are paid for by third parties, those third-party payers often require clients’ consent to collect and disclose to them information that demonstrates their entitlement to this funding or coverage. Discussing what information is shared with third-party payers is an important part of the consent process as well as a topic that can and should be discussed as needed during service delivery, and I encourage you to ask me questions or raise any concerns you may have on this topic.

Disclosure of Your Personal Health Information

Your personal health information will not be disclosed to people outside of this practice without your knowledge and express consent. Written consent is always preferred; however, in time-sensitive situations if you provide verbal consent to share information, written consent can be obtained in our next meeting.

The law requires that any disclosure of your personal health information is limited to information that is reasonably necessary for the purpose of that disclosure and does not include private information provided by a third party. Professional ethical standards governing my practice also require that I not disclose any information that might cause serious harm to someone, unless the law requires disclosure.

Protection and Retention of Your Personal Health Information

The privacy of your personal information is protected through the use of established procedures in my office. Examples of those procedures include that paper and electronic information is secured in a locked or restricted area at all times, computers are password protected. I need to retain your personal health information for some time in order to ensure that I can answer any questions you might have about the services provided as well as for my own accountability to external regulatory bodies.

Your Access to Your Personal Health Information

With only a few exceptions, you have the right to access any record of your personal health information and to request copies of the information (I reserve the right to charge a nominal fee for record copying). If the record contains personal health information about another individual, that person’s information must be severed before you access the record.
